Ontario Health atHome is seeking a Care Coordinator to assess patient needs, develop care plans, and link patients with home and community care. You will work with caregivers and a multidisciplinary team to ensure timely access to services in the Champlain region.

The role offers a 35-hour work week in Renfrew, with a 1.0 FTE, and a starting salary range aligned to hourly rates. Solid English communication and Ontario licensure are required, with French as an asset.
