Dufferin Area Family Health Team is seeking a Registered Practical Nurse – Integrated Care Pathways Coordinator to drive coordinated access, clinical triage, and navigation across priority clinical streams. You will review referrals, connect clients to community resources, and leverage digital systems to close referral loops without dropped handoffs.
You will work alongside Family Physicians, Nurse Practitioners, and regional partners to transform care pathways and support attachment readiness
