Shared Health is seeking a Service Coordinator - ACT to provide case management and supports to participants within the Adaptable ACT team. Under supervision of the Team Leader, you will coordinate intake, assessments and navigation, and ensure alignment of FACTT and ACT components to meet client needs.
The role requires a minimum of four years related community experience with severe and enduring mental illness, and a health-related degree with active regulatory registration.
