Nurse Practitioner Job Description

Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join a highly respected long-term care home in Wiarton on a permanent basis. The home is ideally looking for someone who can join their team permanently. The position is 8 hour shifts, with a bi-monthly on call rotation.

The ideal candidate will have a strong background in patient care, with experience in various settings including paediatrics, senior care, or acute care. As a Nurse Practitioner, you will play a crucial role in providing high-quality medical care to patients, conducting assessments, diagnosing conditions, and developing treatment plans. Your expertise will be essential in delivering comprehensive healthcare services to our diverse patient population.

Duties

- Conduct thorough physical examinations and health assessments for patients of all ages.
- Diagnose and manage acute and chronic illnesses, providing appropriate treatment plans.
- Administer medications and perform procedures as necessary, ensuring adherence to safety protocols.
- Collaborate with healthcare teams to coordinate patient care and optimize health outcomes.
- Educate patients and their families on health management, disease prevention, and wellness strategies.
- Provide case management services to ensure continuity of care across various settings including hospitals, nursing homes, assisted living facilities,



and home care environments.
- Participate in pain management strategies for patients requiring specialized attention.
- Maintain up-to-date knowledge of medical terminology and best practices in nursing.

Qualifications

- Valid Nurse Practitioner license with relevant certifications.
- Strong understanding of dementia care principles, medication administration, phlebotomy and laboratory processes.
- Excellent communication skills with the ability to work collaboratively within a healthcare team.
