Vancouver Coastal Health is seeking a Registered Nurse for centralized intake to coordinate client referrals and initial assessments within a client-centred, continuum-based care model. You will conduct intake screening, triage, and referrals, and provide education to clients and families, while coordinating with the interdisciplinary team.

The role requires evidence-based practice, robust communication, and ability to work within BC provincial standards and program policies.
