Vancouver Coastal Health seeks a Nurse Practitioner to join the Transitional Care Unit (TCU) at Banfield Pavilion. The unit provides care for patients awaiting long-term placement or community services, located on the 4th floor of Banfield Pavilion.

The NP will work with an interdisciplinary team to support patients through critical care transitions. Responsibilities include comprehensive patient assessments, initiating treatments, prescribing medications, guiding other health professionals, and
