POSITION SUMMARY : The Community Mental Health Support Worker (CSW) works under the direction of the Manager/Supervisor, Mental Health Services to provide community based and assertive outreach services.

The CSW applies knowledge related to motivation, community development, and co-occurring mental health and substance use issues, to help the client attain their optimal level of functioning and prevent or postpone further disability, in partnership with the client, the Community Mental Health Worker and the client’s care team.
