London Health Sciences Centre (LHSC) is seeking an Addiction Transitional Case Manager to deliver crisis intervention and brief case management for individuals with substance use health concerns in the Emergency Department. You will connect patients with community addiction and mental health supports and collaborate with a multidisciplinary team.

The role is a full-time, contract position (~4 months) with advantages through HOOPP; evening/night and weekend shift premiums apply.
