We are looking for a highly capable and compassionate Licensed Practical Nurse (LPN) to step into a Care Manager role within our growing Bedford team.

This is not a typical coordinator role—this is a hands-on leadership position at the center of our operations. You will oversee care delivery, support and coach staff, and ensure every client receives exceptional, personalized service.

This role is ideal for someone who thrives in a fast-paced, relationship-driven environment and is confident balancing clinical care, team leadership, and operational oversight.

Key Responsibilities

- Oversee daily operations, including scheduling, staffing, and client service coordination
- Develop, implement, and monitor individualized care plans
- Conduct client assessments and ensure high-quality, person-centered care delivery
- Act as the primary point of contact for clients and families, handling inquiries and complex situations with professionalism and empathy
- Build and maintain strong relationships with clients, families, and care staff
- Lead, coach, and support team members through regular check-ins and performance feedback
- Support hiring, onboarding, and team development
- Perform routine audits and quality assurance checks to maintain high standards
- Oversee and support foot care clinic operations
- Provide hands-on nursing support when required

Qualifications





- Licensed Practical Nurse (LPN) in good standing in Nova Scotia
- Experience in care coordination, case management, or home health care
- Strong organizational skills with exceptional attention to detail
- Proven ability to manage multiple priorities in a fast-paced environment
- Excellent communication and interpersonal skills
- Ability to handle challenging conversations with diplomacy and confidence
- A natural leader with a coaching mindset and strong sense of accountability
- A genuine passion for supporting clients and their families

Nice to Have

- Experience in geriatric and community based care
- Knowledge of local community resources in Nova Scotia
