Care Manager (RN or RPN): Client Services & Care Quality | In-Home Care For Seniors
About Us
Maple Senior Home Care is a recent senior in-home care provider in Ontario. Our goal is to help older adults stay safe, independent, and comfortable in their own homes, with care that families can trust. We’re launching with personal support services and building the clinical foundation to grow over time. This is a chance to shape how care is delivered from day one.
The Role
As our Care Manager, you are the client-services and care-quality lead for the company. You’ll be the first point of contact for families, the person who assesses each client and designs their care plan, the supervisor who keeps our PSW team’s standards high, and the senior clinician accountable for safety and quality. You’ll work directly with the owners and have real authority over how care is delivered.

- Respond to inquiries from families, hospital discharge planners, and referral partners, and explain our services with warmth and clarity
- Screen needs, determine whether a client is suitable for our services, and arrange assessments quickly
- Support smooth admissions and be the family’s ongoing point of contact for concerns
Assessment and care planning
- Conduct in-home assessments covering function, safety, cognition, mobility, nutrition, and home environment
- Develop individualized personal support care plans, and reassess regularly or when needs change
- Recognize changes in a client’s condition and escalate appropriately to the family, physician, or community health partners
- Coordinate with families, physicians, and community partners as needed
PSW supervision and quality
- Serve as clinical lead for our Personal Support Workers, including orientation, competency checks, coaching, and ongoing support
- Conduct supervisory visits and review care notes
- Oversee incident reporting,
follow up on adverse events, and manage complaints
- Develop and maintain care policies and procedures, and support funder reporting and audit readiness
Operations and growth
- Help recruit, screen, and select PSWs alongside the owners
- Contribute to our scheduling and care-documentation software
- Help plan the future expansion of our services
