About Care2Talk Health
Care2Talk Health (care2talk.io) is a Nurse Practitioner-led primary care organization combining virtual and in-person care. Members access care through subscription plans ranging from virtual-only to hybrid in-person access.
We operate clinics in Victoria, Qualicum Beach/Parksville and Edmonton. Our Salt Spring Island clinic extends that model to a community where timely, continuous primary care is hard to find. The LPN is a core member of the island care team.
Role overview The LPN provides hands-on nursing care, supports NP visits, and keeps patient care moving between appointments. The role practises independently within LPN scope and collaborates with NPs on care beyond it.
In a small island clinic, the LPN is often the first clinical contact for patients. Strong judgment, clear communication and comfort working with a lean on-site team are essential.
Key responsibilities
All activities are performed within the LPN scope of practice and the standards, limits and conditions set by the BC College of Nurses and Midwives (BCCNM).
Direct clinical care
- Perform nursing assessments, vital signs and health histories; triage concerns and escalate to the NP as needed
- Administer medications and injections, including immunizations where authorized and competent
- Provide wound care, dressing changes, suture and staple removal, and other routine procedures
- Collect specimens and perform point-of-care testing and ECGs
- Prepare patients and rooms for NP visits and assist with in-clinic procedures
Chronic disease and preventive care
- Support patients living with chronic conditions through monitoring, follow-up and self-management education
- Track preventive and screening care (immunizations, cancer screening reminders) and close care gaps
- Coordinate referrals and follow-up with community services, Island Health programs and Lady Minto Hospital
Virtual and hybrid care
- Conduct nurse-led phone and video follow-ups and pre-visit intake
- Act as the in-person clinician during hybrid visits with off-island NPs, completing exams and procedures on their direction
- Respond to patient messages within defined protocols
Documentation and operations
- Document care accurately and promptly in the electronic medical record (Accuro)
- Maintain clinic supplies, vaccine inventory and cold-chain logs
- Follow infection prevention and control practices and support clinic quality, safety and privacy standards
- Contribute to clinic workflow improvement and orientation of new team members
Required qualifications
- Diploma in Practical Nursing from a recognized program
- Current practising registration as a Licensed Practical Nurse with BCCNM, in good standing
- Current CPR / Basic Life Support certification
- Minimum 2 years of LPN experience, ideally in primary care, community health or a comparable setting
- Competence with electronic medical records and comfort with video and phone-based care
- Clear, compassionate communication and the ability to work independently in a small team
Preferred qualifications
- Experience with Accuro EMR
