You will provide quality, client focused, culturally appropriate, community and outreach based services. You will deliver holistic care that will help young people as they achieve their highest level of community functioning and independence, with the least ongoing professional supports. In this role you will see clients in person at a clinic or community settings, virtually, and by phone to reduce barriers of engagement.
