Qualifications

- Licensed or eligible for licensure in Illinois
- CPR certification
- APTA membership preferred
- Experience treating all ages a plus (from pediatrics to geriatrics)

Job Responsibilities

- Conduct physical therapy evaluations and develop treatment plans
- Deliver evidence-based therapy services
- Supervise PT Assistants as needed
- Collaborate with healthcare professionals for integrated care
- Complete documentation and case management
- Maintain professionalism, empathy, and service excellence
