The Alex Community Health Centre in Calgary seeks a Registered Nurse to join the Pathways ACT Team on a permanent, part time basis. You will deliver medical interventions, education, triage, and timely care coordination for participants facing housing, mental health, and addiction challenges.
You will work within a multidisciplinary team in a community setting, delivering outreach services and collaborating with housing and health partners.
