CMHA Peel Dufferin in Brampton is hiring a Transitional Case Manager for the Brampton HART Hub to provide case management within a Housing First framework. You will conduct psychosocial assessments, develop care plans, link clients to community resources, and support transitions from homelessness into housing and recovery.

The role is full time, based onsite at the Brampton HART Hub, with a multidisciplinary team and opportunities for career development.
