Across Boundaries is a mental health center located at 51 Clarkson Ave., Toronto, ON. We hire a permanent full time Case Manager to support individuals with responsible mental illness, addictions, and criminal justice involvement, serving racialized communities across the Greater Toronto Area.

The role reports to the Program Manager and involves a typical 20–25 client caseload with diverse hours, occasional Sundays, and on-site work. Relevant experience and a commitment to anti-racism are essential.
