ParaMed Home Health Care is seeking a Clinical Planner responsible for onboarding recent patients requiring Personal Support Services in an assigned district. You will assess care needs, design and implement individualized care plans, and evaluate outcomes to ensure safe, high-quality home care.
You will also coordinate scheduling and maintain accurate patient records while respecting privacy and cultural needs.
