Responsibilities Deliver individualized occupational therapy services to residents in a geriatric, skilled nursing environment, following the established plan of care
Assess residents, set goals, develop and implement treatment plans, and document progress and outcomes
Reassess and adjust interventions based on resident response; maintain safety and infection control best practices
Maintain accurate, timely documentation including progress notes, treatment plans, and discharge summaries in accordance with facility policies
Collaborate with physical therapy,



speech-language pathology, nursing, and other team members; communicate with families as appropriate
Manage equipment and therapy supplies; ensure treatment spaces are clean, organized, and safe for resident use
Requirements Active Colorado license as a Certified Occupational Therapy Assistant or eligibility for licensure; NBCOT certification as a COTA
Associate degree in Occupational Therapy Assistant from an accredited program
1 year of experience in a skilled nursing or geriatric setting preferred
Ability to work in office in a skilled nursing facility and adapt to a multidisciplinary team
Solid documentation, communication, and patient safety skills
