Health is seeking a skilled supervisor to lead Community Health Workers in providing essential home support services. You will coordinate care delivery while training and assessing staff for their performance.

Your role will also involve collaboration with an interdisciplinary team to perform client assessments and deliver direct nursing care in accordance with the LPN Standards of Practice. Key Responsibilities:

- Supervise Community Health Workers in home support
- Develop and implement service care plans
- Provide training and evaluate CHW competencies
- Perform client assessments and nursing care
- Collaborate with multidisciplinary teams in service delivery Requirements:
- Registered LPN with BC College of Nurses and Midwives
- Three years related community care experience
- One year supervisory experience preferred
- Valid BC Driver's License
