The Outreach

Worker will execute care plans, assess treatment needs, and facilitate group activities aimed at supporting clients with mental health challenges. Strong communication, teamwork, and crisis management skills are essential for success in this role. Key Responsibilities:

- Assess and evaluate patient care and treatment plans
- Implement care plans and monitor progress
- Provide crisis intervention and support

- Registered with a regulated skilled body
- Two-year diploma in Social Service Work
- Three years of mental health experience required
