The Pharmacist provides comprehensive pharmacy services to patients admitted to Toronto Western Hospital, as part of the patient care team. You will conduct BPMH interviews, reconcile medications, and review orders for safety and appropriateness.
Responsibilities include resolving drug therapy problems, monitoring outcomes, responding to information requests, and supervising technicians while supporting teaching and research within a hospital setting.
