We are looking for RNs and RPNs to work as community nurses servicing patients both in home as well as in the clinc.

We have full time and part time positions available.

Hours of work are inclusive of Weekdays/ Weekends & on-call rotation

Wages

- For Registered Nurses (RN)- This position offers a competitive compensation grid, with progression based on hours worked and role‐specific criteria. The RN per general visit range is $30.20 to $44.58 and the RN hourly range is $30.80 to $45.48. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate's knowledge, skills, qualifications, and relevant experience.
- For Registered Practical Nurses (RPN) - This position offers a competitive compensation grid, with progression based on hours worked and role‐specific criteria. The RPN per general visit range is $24.07 to $29.60 and the RPN hourly range is $24.07 to $29.60. Placement within the grid will be determined in accordance with company policies and procedures,



as well as the candidate's knowledge, skills, qualifications, and relevant experience.

What The Role Involves

- Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health at Home.
- Working with patients (both in-person and virtually) to provide exceptional nursing services
- Providing services in a variety of settings which may include in-home, clinic, retirement home or school
- Communicating and liaising with patients, family members and others within the interdisciplinary care team
- Performing other clinical duties as required

What You Bring

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver's license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- To protect our patients, we require a current vulnerable sector check and a clear background check

In addition to providing home-based health care, Care Partners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we've been organizing staff‐led medical care and clinics in countries with poor access to health care since 2009.
