Supporting resident
admissions, discharges, and transfersCompleting
assessments
and contributing to
individualized care planningAdministering
medications, treatments, and wound careDocumenting care in
PointClickCare




and completing
MDS assessmentsLiaising with
physicians, families, and
interdisciplinary teamsDelegating tasks to care aides and participating in
care conferences
and
medication reviewsWhat You Bring
Active registration as an
LPN in good standing with BCCNMCurrent
CPR and First Aid certificationProficiency in
electronic documentation systems
(e.G., PointClickCare, eMAR)Robust
communication skills
in English (both verbal and written)Effective
critical thinking
and
decision-making
skillsPrevious experience in long-term care
