Job Overview
We are seeking a collaborative, committed NSWOC to join our team for a minimum 25 hours per week and ideally full time. Preference will be given to candidates with experience working with individuals with Spinal Cord Injury and expertise in wound care management.
You will be working with the client, family and the interdisciplinary team to complete comprehensive assessments resulting in recommendations for of wounds for people living with spinal cord injury and other workplace related injuries.
We are looking for an individual based in Greater Vancouver who is willing to travel to outreach areas around the province periodically.
In this role, you will provide assessment and consultation to address physical and rehabilitation needs, with a particular focus on wound care treatment for adults living in the community. You will work closely within a supportive interprofessional team to deliver coordinated, client-centred care that begins with prevention.
Responsibilities
- Liaise with community partners to provide care plans, education and follow up for community clients with wounds. in the Greater Vancouver area and across BC, through in-person, outreach, and virtual telehealth services.
- Work together with the client, family, and their care team to complete wound assessments, provide recommendations, and work collaboratively with OT’s and PT’s.
- Work collaboratively with other health professionals involved in each client’s care to ensure coordinated,
person-centred service delivery.
- Participate in and contribute to formal and informal education related to wound care treatment.
- Prepare transparent, thorough, and timely documentation and reports.
- There is also the opportunity for research projects.
Required Qualifications
- Graduate of an accredited School of Nursing - bachelor’s degree in nursing
- Registered or eligible for registration with BCCNM.
- Completion of Wound, Ostomy & Continence Program through NSWOCC or equivalent( WOCN)
- Membership with Nurses Specialized in Wound, Ostomy & Continence of Canada (NSWOCC) preferred
- Minimum two (2) years of related experience required
- Current Basic Life Support (BLS-C) HCP certification
- Comprehensive knowledge of community resources
- Excellent communication skills (oral and written)
- Demonstrated teaching skills based on principals of adult learning and expertise in preparing effective teaching methods and tools
- Strong caseload management skills and attention to detail
Based in lower mainland with regional travel as required
- BC Driver’s License
- Professional Liability Insurance
Beneficial Experience and Interests
- Strong interest/experience in wound assessment and management experience is an asset, but training and mentoring available
- Experience working with individuals with spinal cord injuries
- Community or residential care experience
