About the Role
This dynamic role provides an opportunity to broaden your clinical skills while supporting patient care across multiple programs, teams, and clinical settings within the Family Health Team.
The Registered Nurse (RN) will work with a variety of patient populations and interdisciplinary teams, providing opportunities to continuously build knowledge, strengthen clinical judgment, and develop adaptability. This role is ideal for an RN who enjoys variety, collaboration, problem-solving, and ongoing professional growth.
As a member of the FHT RN Team, you will play an important role in ensuring patients receive safe, high-quality, timely, and compassionate care where support is needed most.
Key Responsibilities
- Provide comprehensive, patient-centred nursing care across LFHT programs and clinical settings.
- Practice to your full scope of practice, consistent with education, experience, competencies, CNO standards, and organizational policies.
- Complete nursing assessments, interventions, education, counselling, follow-up, and care coordination.
- Support patients with acute, chronic, complex, and preventive health needs.
- Provide nursing procedures and treatments within scope and demonstrated competency, including:
- Immunizations and injections
- Wound care and management
- Blood glucose monitoring
- Ear irrigation
- Suture/staple removal
- Other clinical procedures as required
- Recognize changes in patient condition,
exercise sound clinical judgment, and elevate concerns appropriately.
- Provide health teaching and support patients and families in self-management and healthy lifestyle choices.
- Collaborate with physicians, nurse practitioners, allied health professionals, and other team members to coordinate care.
- Facilitate referrals and connections to internal programs, specialists, and community resources.
- Maintain accurate, timely, and confidential clinical documentation.
- Participate in quality improvement, process improvement, and professional development activities.
- Adapt quickly to changing priorities, clinical environments, and patient needs.
Qualifications
- Current registration as a Registered Nurse with the College of Nurses of Ontario (CNO).
- Diploma or degree in Nursing from a recognized college or university; BScN preferred.
- Primary care, community, acute care, or related clinical experience.
- Strong clinical assessment, critical thinking, observation, problem-solving, and decision-making skills.
- Excellent professional, interpersonal, therapeutic, and patient/family communication skills.
- Demonstrated ability to provide effective patient and family health education.
- Demonstrated competence in nursing procedures appropriate to the role and scope of practice.
- Ability to work independently while recognizing when consultation or escalation is required.
- Strong organizational skills and ability to prioritize competing demands.
- Demonstrated ability to work collaboratively within an interprofessional team.
- Commitment to continued professional development, learning, and practice improvement.
- Basic computer literacy and ability to learn and use electronic medical records and clinical applications.
- Knowledge and adherence to Occupational Health and Safety, infection prevention and control, privacy, and organizational policies and procedures.
- Commitment to providing care with courtesy, compassion, professionalism, and respect.
Compensation package includes
- $34.00-$37.00 (based on experience)
