Humber River Health is seeking a Dialysis Liaison to join the Nephrology Program. The role focuses on organizing and providing home haemodialysis treatment within the HRH catchment, aligning with patient needs, time, supplies, and equipment in collaboration with the registered nurse.
The position is permanent part time, with rotating shifts including days/evenings/weekends. Must have a Personal Support Worker certificate or equivalent, valid driver's license, and CPR/BCLS.
