This part time role supports veterans within the District of Thunder Bay by confirming services through Veterans Affairs and directing them to the Veteran Homelessness Program. You'll coordinate diverse supports to minimize homelessness risks and enhance self-sufficiency among veterans.
Your prior experience in case management and community services will be invaluable in this role.
Key Responsibilities:
Assess and confirm services for veterans
Coordinate supports via the Veteran Homelessness Program
Help participants achieve self-sufficiency goals
Relate sensitively to vulnerable individuals
Facilitate relationships with community specialists
Requirements:
Social Work Degree or equivalent diploma
Minimum three years in service coordination
Knowledge of Veterans Affairs Programs
Excellent communication skills and conflict resolution
Valid driver's license and insurance
Utilize your experience in case management to support veterans and improve their quality of life in Thunder Bay.
