KeyResponsibilities:
Assist individuals innavigating healthcare services and resources.
Provide education on healthtopics and disease prevention.
Conduct outreach and supportcommunity health initiatives.
Collect data on communityhealth needs and barriers to access.
Advocate for patients and connect them withnecessary services.
Requirements
High school diploma;relevant certification or degree preferred.
Experience in communityoutreach or healthcare settings.
Robust communication andinterpersonal skills.
Cultural competence and understanding ofcommunity dynamics.
