Position: Integrated Support Worker, CI Housing

Hours of Work: Minimum of 12 hours per week. Days, Evenings, Overnights, Weekends.
Reports to: Clinical Program Manager, CI Housing
Location: Kingston
Hourly Rate: Pay Band 5, $ 30.23 - $ 35.36
Job Status: Permanent Part-time

Job Summary: The Integrated Support Worker (ISW) works as a member of an inter-skilled team carrying out rehabilitation and support functions in a community living environment. This position collaborates closely with the clinical team to provide side-by-side support and life skills teaching for individuals 16 years of age and older living with a serious mental illness and/or addiction, within the high support of a transitional housing program. The incumbent is responsible for supporting the delivery of individualized treatment plans, as well as advocacy and crisis intervention. The incumbent is expected to support community integration including assistance with transportation and attendance of appointments outside the work location.



All services are provided in accordance with the mission, vision, values, and established protocols of the Agency.
There are a number of specialty Integrated Support Worker roles, supporting clients across multiple programs at the Agency. Further information on this variation of the role:
Comprehensive Integrated Housing (CIH) : This position works in a 24-7 transitional treatment program, providing daily living support and life skills teaching to individuals living with a serious mental illness, who may have a history of substance use. This position works collaboratively within a multi-disciplinary treatment team and with community partners to deliver individualized treatment plans, with the goal of graduation to reduced support and transition to independent living in community, over a two-year timeframe. As an important part of the team, this role is a float position designed to provide coverage across various shifts, with a guaranteed minimum of 12 hours per week. Additional hours may be available based on staff needs and availability within the housing program.
