Physically located in Toronto, The 519 is the largest community centre of its kind, committed to the health, happiness and full participation of the Two-Spirit, Queer, Trans, Non-Binary and Gender-Diverse (2SQTNB+) communities. All directly operated programs and services at The 519 including weekly drop-ins, community food initiatives, housing and shelter support, newcomer settlement services, child and family development, youth and elder programs, legal and tax clinics, access to justice programming, education, training, community organizing and advocacy, focus on the needs of the local and broader 2SQTNB+ communities.

Ontario’s Justice Centres
Ontario’s Justice Centres are an innovative court model that integrate early supports into the justice process by bringing together justice, health, employment, education, and social services to address the root causes of crime, break the cycle of offending, and improve public safety and community well-being. Toronto’s Downtown East Justice Centre focuses on breaking the cycle of offending by addressing the root causes of crime for accused at the intersection of poverty, homelessness, mental health and addictions challenges. Toronto’s Youth Justice Centre focusses on supporting youth (aged 12-17) charged under the Youth Criminal Justice Act (YCJA).
PRIMARYFUNCTION
ReportingtotheManagerofLegalInitiatives,theCommunity Case Manager,AccesstoJusticeposition is an experienced social services worker with strong knowledge of legal issues affecting 2SLGBTQ+ (Two-Spirit, Lesbian, Gay, Bi, Trans, Queer+) communities and excellent skills in client services engagement and navigation of the criminal legal and social services systems. Community outreach is a key part of the position.
In the position, theCommunity Case Manager, willprovideenhancedwrap-aroundservicesandsupportsforreferred 2SLGBTQ+ Justice Centre and The 519 women and gender-diverse participants, including justice-involved youth and adults.
This role will be primarily based out of 35 Britain Street, and requires active and collaborative engagement with the DTE JC team which includes Ministry of the Attorney General Crown Counsel, Case Management Coordinator, and Case Managers from partner agencies including Regent Park Community Health, Surrey Place, the Jean Tweed Centre for Women and LOFT 416 Community Drop In for Women.
KEYACCOUNTABILITIES
Provideintensivecasesupportfor2SLGBTQ+JusticeCentre(JC)participantsfromthe DowntownEastandYouth courtsites, as well as The 519,toaddressidentifiedneedsandstrengths in areas including, but not limited to, mental health, addictions, housing, education, employment, practical supports, and pro-social activities.
SupportthedevelopmentofanIndividualPlanforreferred2SLGBTQ+JCparticipants navigatingthecriminal(asaccused,victimorwitness),family,healthorhousinglegal systems in time-limited systems-navigation and communications supports. Supports may include supporting the participants’ existing relationships with community-based organizations, accompaniment to hearings and meetings with lawyers, assist with completingforms,assemblingpaperwork,aswellasassistingclientswhoneedsupport accessing social, educational and health services such as obtaining identification, employment,recreation,accompanyingtoappointments,andothersasrequiredbythe Individual Plan.
Delivertrauma-informedandresponsivecasemanagementservicesforreferred 2SLGBTQ+ JC, and The 519 participants
ConnectwiththereferredJC or The 519participant,helpthemmaintainreleaseconditions and work to achieve individual and community wellbeing goals
Increase the awareness of right to counsel for justice-involved individuals
Supportthe preparation and progress of participant’sIndividualPlans
Attend/participateincourtdatesandconductscreeningassessmentsin partnership with JC staff for participants
AcceptandtrackreferralsbasedontheneedsofreferredJC participants
ProvideinputtoJCTeamonindividualcircumstances,ongoingengagementand willingness of the JC participants to engage with supports, services, andprogramming
Providesystemsnavigationincludingcourtaccompaniment,administrative support, and advocacy while navigating social, legal and health services
MaintaintheconfidentialityandsecurityofJCparticipants’personalinformationin accordancewithapplicablelegislationsuchasFreedomofInformationandProtectionof Privacy Act, the Municipal Freedom of Information and Protection of Privacy Act, and the Personal Health Information Protection Act, the Youth Criminal Justice Act, The 519’s privacy policies and procedures, and the industry’s best practices.
Gain consent prior to obtaining personal information, keeping information secure, restricting access to personal information of JC participants to only those that need access,disclosinginformationonlyasrequired,andpromptlyreportinganybreaches.
Workcloselywithotheragenciestosupportparticipantsandtheirfamiliesasneeded, including supporting participants’ existing relationships with other community-based organizations.
DevelopreferralpathwaysforjusticeinvolvedindividualsassignedtoThe519
IncollaborationwithotherJCstaffmembersandtheJCparticipants’needs assessment, design referral pathways and partnerships to support the participant.
Identifyin-kindsupportservicesandpartnershipssuchasaccesstotraining, health services, coaching or peer programs. Support the development of collaborativein-houseprogramswithothercommunityagencies.
PartnerwiththeMinistryoftheAttorneyGeneralandtheCityofTorontostafftoco-lead opportunities to co-design strategies better support 2SLGBTQ+ BIPOC Justice-involvedindividuals.Providesubjectmatterexpertise,co-facilitationandstrategicinput into the design of a Justice Centre programs to support 2SLGBTQ+ BIPOC justice-involved individuals in a sustainable and ongoing way.
Participateinweeklymeetings,courtbriefingsandanyandallcourtappearancesforJC participants.
Monitor,evaluateandreporton outcomes
Maintainfilesandrecordsofparticipants’activitiesincludingbutnotlimitedto documentation related to the Needs Assessment, the Individual JC Plan Template,andreferralsmadefortheJCparticipants.

REQUIREDKNOWLEDGEANDEXPERIENCE
Apost-secondarydegreeinsocialworkorarelatedfield.

Experience facilitating muti-sectoral needs assessment and making needs-based, strengths-based, and trauma-informed connections to support, services,
