Join Fraser Health as a Licensed Practical Nurse Outreach, providing critical care for clients with mental health disorders. This role involves working both in the community and at home, focusing on patient-centered support.
As a member of a multidisciplinary team, the LPN Outreach conducts assessments, administers medications, and plans patient care collaboratively with RNs and RPNs. You'll be responsible for monitoring patient needs and contributing to care plans effectively. Your skills in organization and communication will enhance patient outcomes and team collaboration.
Key Responsibilities:
• Administer medications following policies and procedures
• Assess and monitor patient strengths and limitations
• Contribute to client care plans and interdisciplinary meetings
• Organize patient care and respond to changing needs
• Document care information to ensure patient safety
Requirements:
• Graduate from a recognized Practical Nursing program
• Current LPN registration with BCCNM
• First Aid, CPR, and Food Secure certifications
• Valid BC Driver's License and access to a vehicle
• Ability to communicate and work independently
Deliver personalized nursing care and participate in quality improvement initiatives as part of Fraser Health's dedicated team.
