This role focuses on outreach to clients referred by Community Living British Columbia. You will create tailored intervention plans addressing daily living aspects, including housing, nutrition, and employment. Flexibility is key, as shifts vary based on client needs, including evenings and weekends.
Key Responsibilities:
• Assess client needs through detailed background research
• Collaborate on intervention plans with measurable goals
• Ensure daily living issues are addressed in plans
• Support clients in various challenging environments
• Communicate with other professionals to align services
Requirements:
• Diploma in Social Services and one year of experience preferred
• Experience with vulnerable populations facing barriers
• Ability to respond effectively in crisis situations
• Valid class 5 driver’s license and personal vehicle
• Emergency First Aid Certificate required
