As a key member of the Patient Services team, the Field Nurse Case Manager will coordinate and deliver services associated with patient support programs. This role includes being the primary point of contact for healthcare providers and ensuring seamless access to support programs for patients. You will conduct patient enrollments, gather necessary information, and maintain communication to optimize treatment outcomes.

Key Responsibilities:
• Adhere to Sentrex Policies and Procedures
• Manage HCP and patient registrations




• Process data entry for program services
• Provide valuable medical support and education
• Conduct case monitoring and therapy adherence support

Requirements:
• Registered Nurse in valuable standing
• 2-4 years of clinical experience
• Experience in Patient Support Programs
• Oncology, gastro or rheumatology experience
• Bilingual in English and French is an asset
