London Family Health Team – Intake Coordinator – Primary Care
London Family Health Team (LFHT) is seeking a highly organized and collaborative Intake Coordinator to support the implementation of patient attachment and initiatives aligned with Ontario’s Primary Care Interprofessional Team (IPCT) priorities.

Location: Onsite – possible multiple FHT sites
The Intake Coordinator plays a key role in welcoming and onboarding new patients into primary care. This position serves as the first point of contact for patients entering the practice and ensures that the intake process is timely, organized, patient-centred and coordinated with the primary care team.
Working closely with physicians, nurses and allied health professionals, the Intake Coordinator supports patients from initial application or referral through to their first primary care appointment and connection with appropriate services.
Key responsibilities include:
Contacting and onboarding new patients from referrals, online applications and other access pathways.
Confirming patient information, eligibility and required documentation.
Coordinating initial and follow-up appointments.
Supporting patients in understanding the onboarding process and next steps.
Identifying barriers to accessing care and connecting patients with appropriate resources.
Coordinating warm handoffs to FHT services and community supports when appropriate.
Maintaining accurate intake documentation and EMR records.
Supporting attachment and patient-access reporting.
Identifying opportunities to improve the patient onboarding experience and practice workflows.
Qualifications:
Education in health administration, medical administration, health care, social services or a related field.
Experience in primary care or a health-care setting is preferred.
Excellent communication, organization and interpersonal skills.
Clinical skills/knowledge (an asset)
Strong computer and EMR skills, preferably Telus Practice Solutions Suite (PSS) (an asset)
Ability to manage competing priorities and work effectively as part of an interdisciplinary team.
Demonstrated commitment to patient-centred, equitable and accessible care.
NOTE:
This is an administrative/patient-navigation role and does not involve clinical assessment or diagnosis. Clinical concerns identified during intake are escalated to the appropriate regulated health professional.
Contract Terms:
Hours: Up to 40 hours per week (to be negotiated)
Rate: $25.00 – $28.00 per hour
