Make a difference as a Case Worker with The Salvation Army, delivering participant-centered case management for vulnerable individuals and families. Support clients in crisis, helping them achieve stability and independence through community resources.
As a valued Case Worker, you will leverage your skills in social services and case management to assist clients in goal setting, action planning, and crisis navigation. Your role includes developing trusting relationships, conducting assessments, and maintaining an organized caseload according to Salvation Army policies. Work collaboratively with community partners to ensure holistic support.
Key Responsibilities:
• Conduct participant intake and assessments regularly • Develop and implement individualized action plans • Provide referrals and advocacy for community services • Maintain confidential and detailed participant documentation • Support clients in crisis and navigate social service systems
Requirements: • Two-year post-secondary diploma in human services • Experience in case management or social services preferred • Strong interpersonal and relationship-building skills • Proficiency in Microsoft Office and electronic documentation • Willingness to obtain required certifications, including First Aid
