PRIMARY FUNCTION

The Nurse Practitioner (NP) provides advanced clinical care to adult patients within the Thrombosis and Inherited Bleeding Disorders Programs. Working as an integral member of an interdisciplinary hematology team, the NP provides comprehensive assessment, diagnosis, treatment, monitoring, education, and follow-up for patients with venous thromboembolism (VTE), inherited bleeding disorders, and related hematologic conditions.

The NP must know and practice in accordance with the standards relevant to the practice area as well as the legislation and regulations applicable to an NP practice:

- Regulated Health Professions Act, 1991;
- Nursing Act, 1991;
- Laboratory and Specimen Collection Centre Licensing Act;
- Healing Arts Radiation Protection Act, 1990;
- Public Hospitals Act, 1990;
- Health Protection and Promotion Act, 1990.

The NP functions to improve timely access to care, enhance patient outcomes, reduce delays in treatment initiation, support care transitions, and provide continuity across the patient journey. The role combines direct patient care, clinical consultation, care coordination, quality improvement, and interdisciplinary collaboration.

This position is divided approximately equally between the Thrombosis Program and the Inherited Bleeding Disorders Program.

RESPONSIBILITIES & DUTIES INCLUDE

Acute Venous Thromboembolism (VTE) Triage and Management

Provides rapid clinical assessment and follow-up for newly referred patients with acute VTE, including:

- Reviewing referrals and diagnostic investigations.
- Conducting timely telephone assessments following referral.
- Confirming diagnosis and treatment plans.
- Assessing symptom burden, bleeding risk, and clinical stability.
- Reviewing anticoagulant therapy for appropriateness, adherence, dosing, and safety.
- Monitoring laboratory investigations, including renal function and other relevant testing.
- Identifying patients requiring expedited physician review.
- Providing patient education regarding anticoagulation management, safety, and follow-up.

Acts as a clinical resource to support early intervention and reduce delays in access to specialist care.

Ambulatory Thrombosis Clinic

Provides independent and collaborative NP-led follow-up care through a dedicated ambulatory thrombosis clinic, including:

- Assessment and management of patients with VTE.
- Anticoagulation monitoring and optimization.
- Ongoing surveillance and follow-up.
- Development of individualized care plans.
- Coordination with primary care providers and community partners.

Inherited Bleeding Disorders Program (0.5 FTE)

Outpatient Procedure Planning and Hemostatic Management

Acts as the primary clinical coordinator for adult patients with inherited bleeding disorders undergoing elective outpatient procedures.

Responsibilities include:

- Receiving referrals and notifications regarding upcoming procedures.




- Completing clinical assessments and bleeding risk reviews.
- Collaborating with hematologists, residents, and interdisciplinary team members to develop individualized peri-procedural hemostatic plans.
- Coordinating treatment recommendations.
- Writing patient care plans and provider communication letters.
- Ordering and facilitating required therapies include:
- Tranexamic Acid (TXA)
- Ensuring patients understand their treatment plans.
- Conducting post-procedure follow-up to assess outcomes and identify complications.

Iron Deficiency Assessment and Management

Provides ongoing assessment and management of iron deficiency and iron deficiency anemia in patients with inherited bleeding disorders.

Responsibilities include:

- Ordering and interpreting laboratory investigations.
- Initiating and monitoring oral iron therapy.
- Coordinating intravenous iron treatment when indicated.
- Monitoring treatment response and patient outcomes.
- Providing patient education regarding iron replacement therapy.

Inherited Bleeding Disorders Follow-Up Clinic

Provides NP-led follow-up care for established adult patients with inherited bleeding disorders, including:

- Clinical assessment and symptom management.
- Review of bleeding events and treatment effectiveness.
- Monitoring treatment adherence and outcomes.
- Coordination of ongoing care plans.

Clinical Triage and Coverage Support

Provides clinical support and back-up coverage for the Inherited Bleeding Disorders Program as required, including:

- Telephone assessment of acute bleeding concerns.
- Clinical triage and determination of appropriate level of care.
- Administration of DDAVP and clotting factor replacement therapy in urgent situations.
- Coordination with hematologists and emergency services when escalation is required.
- Supporting continuity of care during nursing absences.

Leadership and Collaboration

The NP:

- Collaborates closely with hematologists, residents, nurses, pharmacists, laboratory services, and community providers.
- Participates in quality improvement, program development, and clinical practice initiatives.
- Supports evidence-based practice and implementation of clinical guidelines.
- Provides mentorship and education to learners and staff.
- Contributes to the development of clinical pathways, policies, and procedures.

BASIC QUALIFICATIONS

- Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse in the Extended Class (RN(EC))
- Baccalaureate Degree in Nursing (BScN)



from an Ontario University or equivalent.
- Masters or Doctoral degree in nursing with a clinical specialty from a Canadian University or equivalent.
- Minimum two (2) years’ experience as a Nurse Practitioner preferred. Experience in hematology, thrombosis, and/or inherited bleeding disorders preferred.
- Demonstrated advanced clinical knowledge and practice at an expert level.
- Experience working in Thrombosis and Inherited Bleeding Disorder Programs
- Certification in a relevant clinical specialty (e.g., CNA or equivalent).
- Experience in an ambulatory care setting.
- Excellent communication skills (written and verbal) and strong interpersonal abilities
- Demonstrated ability to work collaboratively within an interdisciplinary team
- Robust organizational skills with the ability to manage competing priorities
- Commitment to patient centered care, quality, and positive patient outcomes.
- Ability to work effectively as a team member.
- Commitment to standards of excellence and accountability
- Knowledge of relevant legislation and professional standards including:

- Proficiency with Microsoft Office applications (Word, PowerPoint, and Excel) and clinical information systems (e.g., EMR).
