As a Registered Nurse at SE Health, you will provide comprehensive assessments and develop personalized care plans tailored to client needs. Key Responsibilities:
• Complete detailed client assessments and care evaluations
• Develop and implement tailored care plans based on best practices
• Serve as point of care lead for client caseload management
• Promote client transitions by connecting them with community services
• Collaborate with care team members to ensure comprehensive care
Requirements:
• Current registration with the College of Nurses of Ontario
• Enthusiasm for community care and evidence-informed practice
• CPR certificate and valid driver’s license required
• Experience with lifting and physical demands of the job
• Willingness to complete Saint Elizabeth certifications
