The Community Health Centres of Northumberland (CHCN) is a non-profit, registered charity that has been providing comprehensive, community-based health services since 2008. Established in 2007 and governed by a Community Board of Directors of twelve dedicated local residents, CHCN reflects the diverse strengths, experiences, and perspectives of Northumberland County.

CHCN’s interdisciplinary team of approximately 65 staff includes primary care providers, allied health professionals, dentists, dental hygienists and assistants, community health workers, and social workers. Together, they deliver care through an integrated model of health and wellbeing that recognizes the social, economic, and environmental factors influencing health.

The Community Health Centres of Northumberland Diabetes Education Program (CHCN DEP) provides free Foot Care Services to clients who are unable, due to financial constraints, to access these services elsewhere.

Demonstrates advanced foot care skills, including comprehensive foot assessment, routine and advanced foot care procedures, infection prevention, and safe use and maintenance of foot care equipment.
Demonstrates strong communication, client education, and interpersonal skills to support diabetes self-management and collaborate effectively with clients, families, primary care providers, and the interdisciplinary diabetes team.

KNOWLEDGE
Applies current knowledge of diabetes, diabetic foot complications, wound prevention, infection control, and evidence-based foot care practices to provide safe, client-centred care.
Maintains knowledge of CNO Standards of Practice, organizational policies, documentation requirements, and applicable Ontario Health reporting and infection control standards.

JUDGMENT
Uses clinical judgment to assess clients' foot-care needs, identify potential risks or complications, determine appropriate interventions,



and recognize when referral or follow-up with the primary care provider is required.
Exercises sound judgment when determining client eligibility for subsidized foot care services and assists clients who do not qualify in accessing appropriate alternative community resources.

ACCOUNTABILITY
Takes responsibility for providing safe, ethical, and high-quality foot care while maintaining accurate, timely documentation and protecting client confidentiality.
Is accountable for maintaining professional competence, following CNO and organizational standards, completing required reporting, and ensuring supplies, equipment, infection control practices, and client care processes meet established standards.

RESPONSIBILITIES
Conducts all aspects of foot care services in accordance with the highest quality care
Standards of Practice established by the CNO
Ensures all supplies and related equipment are properly maintained, cleaned and available
Ensures appropriate infection control standards are followed in relation to the CNO, MOHLTC and CHCN
Maintains accurate and timely client records and documentation in relation to CNO Practice Standards and CHCN policies
Collaborates with Diabetes Education Team, and works together with client, team and primary care provider to ensure comprehensive care and communication regarding foot care
Provides health teaching for best possible diabetes foot care management/self-management
Reviews and evaluates clients’ CHCN Financial Means Assessments in order to determine appropriate clients and facilitates access to other foot care services for those not qualified




Collects and reports foot care data for Ontario Health reporting, in accordance with the predetermined schedule
Conduct occasional foot care outreach activities, as developed by the Diabetes Education Program
Accountable for being knowledgeable of all policies and procedures related to their employment and work environment
Promotes the Diabetes Foot Care Program as part of the overall Diabetes Education Program mandate
Registration in valuable standing with the College of Nurses of Ontario (CNO) RN/RPN
Minimum 2 years’ recent experience as a practicing foot care nurse (preference to those with diabetes foot care experience)
Advanced Foot Care Certificate (proof will be required)
Experience with clinical electronic medical record/scheduler an asset
Ability to work independently and collaboratively in an interprofessional environment
Current driver’s license and access to a reliable vehicle and a willingness to travel
Completion of a Vulnerable Sector Police Check will be required for employment
