GENERAL ACCOUNTABILITY:
In collaboration with health system partners and clinicians, the Navigator acts as a clinical resource and liaison providing support,leadership,direction,andeducationtofacilitatethecoordinationandcontinuityof servicesin matters related to the patient.This position reports to the Provincial Manager, Systemic Therapy and Outpatient Services.
QUALIFICATIONS:
Education:
Bachelor’s degree or Diploma in Nursing
Completion of the Oncologynursingcertification CON(C) or equivalent. Where this is not complete the successful candidate is required to obtain certification within two years of date of hire to the position.
Membership:
Licensed with the Saskatchewan Registered Nurses Association (SRNA).
Experience:
Demonstratedexperienceinleadership and/or programdevelopmentandevaluationand/orpatient education is required.
Minimum 3 years of Disease Site and/or Navigator and/or Educator and/orOncologynursing experienceis required.
For certain navigator positions, experience living or working in a specific geographic area and/or proficiency in a particular dialect or language may be considered an asset.
Skills :
Demonstratedabilitytoworkcollaborativelywithmultiplediversestakeholdersatalllevelsof responsibility both within the SCA and externally.
Demonstratedabilitytoprovideperson-centered,traumainformed,culturallysafenursing support.
Advanced patient assessment skills,
virtually and in person, with the ability to complete initial and ongoing patient assessments(clinicalanddiagnosticreasoning)andprovide highqualitypatient supportthrough appropriate/prescribedtechnical,therapeuticandsafety interventions.
Abilitytointerpretdetailedinformationandcommunicateeffectivelybothorallyandin writing.
Requireshighdegreeofjudgment andproblem -solving abilityandcriticalthinkingskillsto identify, analyze and define problems and determine the most appropriate pathway.
Abilitytointegraterelevantevidence-basedbestpracticeasstandardperformance .
Abilitytoteachandpresentinformation effectivelytopersonsoverwhomtheincumbentmay have no direct authority.
Requireshigh-levelskillsinhumanrelationstodevelopformalandtrustedrelationshipswith patients to navigate them effectively within the provincial health system and provide in-depth counselling and support.
DemonstratedleadershipskillsrequiredtoleadAgency projects/committees.
Abilitytomanage , prioritize andtroubleshootmultipletasksinvariousstagesofcompletion .
