About CHS (Community Health Services)
Community Health Services (CHS) offers barrier-free,culturally responsive health services to immigrants, refugees, and low-incomeindividuals. CHS combines primary care access, healthcare navigation andadvocacy, educational workshops, and mental health counselling and is deliveredin collaboration with a network of trusted healthcare partners.
Position Summary:
The Community Health Navigator and Coordinator works closelywith MNLC's Community Health Services (CHS) team and healthcare partners tocoordinate and support client-centered programs and services and promote healthand wellness in the community. Reporting to the CHS Director, this rolesupports system navigation, facilitates communication with healthcareproviders, and offers advocacy as well as interpretation services for clientswith limited English proficiency. It is ideal for someone who is detail-oriented,personable, and thrives in a fast-paced, client-centered environment.
IMPORTANT: Fluency in both spoken and written English and Spanish is a mandatory requirement for this position. Candidates who are not fluent in both languages will not be considered.
Responsibilities:
Professionally and warmly greet clients in person, byphone, or via email. Monitor and respond to CHS emails and voicemails on adaily basis to ensure timely communication.
Conduct intake screenings to better understand the type ofservice required, including identifying primary health concerns, and/orreferring to emergency care (e.g., ER) if necessary.
Handle internal and external referrals and connect clientsto appropriate services by utilizing existing partnerships and identifying newresources based on each client’s specific needs.
Contact clients to provide and confirm appointmentdetails, including date, time, and location, and send follow-up confirmationsand reminders via email.
Provide individualized support to individuals seekingaccess to health and social services, including offering in-person assistanceto clients who arrive without appointments.
Respond to client inquiries about MNLC services andexternal programs, including topics such as refugee claims and humanitarianapplications. Support clients in accessing a wide range of services inNorthwestern Toronto,
such as health care, housing, employment, education, lifeskills, and community support programs.
Collaborate closely with MNLC's Mental Health team toidentify and support eligible clients who may benefit from mental healthservices. Facilitate smooth referrals and follow-up to ensure clients receiveappropriate MH support.
Facilitate clear and accurate interpretation betweenclients and healthcare providers without offering personal opinions orinfluencing health-related decisions.
Follow up with service providers and advocate on behalf ofclients when necessary to ensure their needs are met.
Facilitate clients' access to on-site donated items orfood if available.
Support student nurses in navigating settlement and legalservices with clients.
Population Health Support
Maintain a detailed list of clients interested in servicesfrom healthcare partners.
Collaborate with healthcare partners to coordinateservices and plan workshops tailored to the community’s diverse needs.
Coordinate clinic visits by confirming service themes,scheduling appointments, assigning time slots, and sharing client details andservice needs with external partners.
Proactively develop new partnerships to expand serviceoptions, considering eligibility, immigration status, and partner organizationofferings.
Work with the Communications team to support outreach andpromote CHS events, workshops, and health initiatives.
Assist in preparing materials for orientation sessions,workshops, and client appointments, including creating Spanish-languagepromotional content as needed.
Attend health-related events alongside Communicationsstaff when needed.
Data Management & Reporting
Create and update client profiles in client managementsystems (e.g., Apricot, Excel, SharePoint) to support timely and accurateinternal or external referrals.
Ensure accurate maintenance of paper and digital recordsin line with confidentiality policies and support the collection andverification of required intake documents.
Collect program statistics as required for reporting andplanning purposes, and report on program challenges and successes.
Facility Coordination
Ensure a safe, private, and welcoming workplace thatsupports client confidentiality.
Maintain clinic readiness for partner visits by ensuringcleanliness, temperature control, and overall accessibility.
Order supplies and coordinate vendor deliveries or pick-ups.
Support site operations as needed (e.g., front deskcoverage, printing, form updates).
Other
Actively participate in regular team meetings and trainingsessions
Contribute to a collaborative and inclusive workplaceculture aligned with MNLC’s values.
Coordinate volunteers who assist with events and provideinterpretation or sight translation.
Handle other related duties as assigned
Qualifications:
Post-secondary education in social services, communitywork, health related-fields or equivalent combination of education andexperience.
1 – 2 years of experience in a customer service,administrative, or community-facing role (preferably in a non-profit orsettlement setting)
Experience navigating systems (e.g., healthcare, legal,immigration)
Experience working collaboratively with partners within thecommunity
Experience working within an anti-racism/anti-oppressiveframework while providing direct support to vulnerable individuals.
Strong knowledge of resources within northwestern Toronto.
Fluent in both spoken and written English and Spanish, withthe ability to accurately interpret between the two languages; proficiency inadditional languages is an asset.
Proficient in navigating relevant systems with knowledge ofrefugee claims and humanitarian and compassionate applications.
Knowledge of systemic barriers, including those related todomestic violence, anti-human trafficking, and the impacts of invisibility andprecarious immigration status, is highly valued.
Understanding of trauma-informed care and how to apply itin frontline interactions.
Proficiency with Microsoft Office (Word, Excel, Outlook)
Understanding the strengths and challenges of communityresidents, along with strong client management skills.
Strong interpersonal and communication skills, with theability to engage empathetically and respectfully with clients from diversebackgrounds while maintaining discretion, emotional regulation, and aprofessional, non-intrusive presence.
Experience coordinating programming and staff or volunteers
Self-motivated and creative with strong organizationalskills, attention to detail, and the ability to manage time and taskseffectively both independently and within a team.
Demonstrated ability to maintain strict confidentiality andprotect sensitive client information.
This role requires a five-day workweek, with at least threedays spent on-site at the office or participating in community events.
