About the Role Our client is seeking an experienced Community Health Navigator to join their team in Kimberley , operating under a hybrid work model. This crucial role involves connecting individuals and families within the community to essential health services and resources. You will provide guidance, support, and advocacy to help clients navigate the healthcare system, overcome barriers to care, and achieve better health outcomes. The hybrid nature of this position allows for both direct community interaction and remote administrative and coordination duties, ensuring comprehensive support for the residents of the Northern Cape .
Key Responsibilities Assist clients in identifying their health needs and navigating the healthcare system.
Provide information about available health services, programs, and insurance options.
Help clients overcome barriers to accessing care, such as transportation, language, or financial constraints.
Build and maintain relationships with healthcare providers, community organizations, and social service agencies.




Conduct outreach activities to raise awareness about health services within the community.
Document client interactions, progress, and outcomes accurately.
Requirements Bachelor's degree in Public Health, Social Work, Nursing, or a related field.
Previous experience in health navigation, community health, or social services.
Robust knowledge of the local healthcare landscape and community resources.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work independently and manage a hybrid schedule, including remote work and community visits.
Proficiency in using client management systems and standard office software.
