Extendicare is seeking a dedicated Licensed Practical Nurse to deliver quality patient care in a part time role. This position focuses on patient assessments, care planning, and direct resident support.
As a member of Extendicare's interdisciplinary team, the Licensed Practical Nurse will implement and evaluate individualized care plans in a senior's care environment. Familiarity with gerontology and strong communication skills are essential, as you will work closely with residents and their families. The role demands adherence to regulatory standards, medication administration, and documentation in alignment with Extendicare policies.
Key Responsibilities:
• Collaborate with the care team for effective patient assessments
• Implement and evaluate resident care/service plans
• Provide direct care and medication administration
• Educate residents and families on care practices
• Respond to emergencies and manage significant health changes
Requirements:
• Current registration as a Licensed Practical Nurse
• Experience in a senior's environment preferred
• Strong time management and critical thinking skills
• Basic Cardiac Life Support and First Aid certification
• Willingness to undergo a Vulnerable Sector Check
