Using motivational interviewing and behaviour‑change approaches, the counsellor offers individualized support through in‑person sessions, phone, and virtual platforms.The successful candidate will play a key role in program development, helping establish workflows, community partnerships, referral pathways, and operational processes. Strong independent leadership, initiative, and communication skills are essential.This position is based primarily at St. Joseph’s Healthcare Hamilton, and will require occasional travel to Hamilton General Hospital.ResponsibilitiesSmoking Cessation Counselling & Patient SupportProvide supportive counselling to individuals quitting smoking or vaping through in‑person, phone, live chat, text‑to‑chat, or email modalities.Use motivational interviewing and behaviour‑change models to develop tailored quit plans aligned with patient goals.Create a supportive, respectful environment that encourages engagement and behaviour change.Provide evidence‑informed cessation information and resources.Complete referrals to community services, care teams, or specialized clinics when needed.Program Leadership & Development (New Clinic)Contribute to the development and implementation of a new regional Smoking Cessation Clinic, including workflows, documentation standards, care pathways,
and communication tools.Support program planning, evaluation, and quality‑improvement initiatives with clinical leadership.Assist in creating patient education materials, orientation tools, staff and patient resources.Participate in developing outreach strategies and promoting cessation services within the partner organizations.Operational CoordinationReceive referrals, triage for completeness, and coordinate appointment scheduling according to clinic protocols.Communicate with patients to schedule appointments, provide instructions, and answer operational questions.Collaborate with interdisciplinary care teams and partner hospitals to ensure smooth service delivery and patient flow.Maintain accurate, timely documentation and follow all privacy, data‑entry, and communications protocols.Inter‑Site Travel & CollaborationTravel to partner hospitals to provide counselling services, staff support, education, and coordination activities as required under the MOU.Represent the clinic in meetings and collaborative planning sessions across the participating organizations.QualificationsRequiredPost‑secondary education Counselling, Health Promotion, Respiratory Therapy or a related discipline.Must have and maintain current BCLS (Basic Cardiopulmonary Life Support)3 to 5 years experience providing client‑centred counselling, health teaching, or behaviour‑change support.Training and competency in motivational interviewing and behaviour‑change counselling.Strong verbal and written communication skills across in‑person, phone, and virtual channels.Ability to organize referrals,
coordinate bookings, and manage clinic communications.Competence with digital platforms, documentation systems, and virtual service tools.Valid Ontario driver’s licence and access to reliable transportation for inter‑site travel.Preferred / AssetsCertified in Training Enhancement in Applied Counselling and Health Teaching (T.E.A.C.H); or near completion of training.
