Your role will involve coordinating and implementing detailed discharge plans, ensuring continuity of care through effective communication with all stakeholders. This position is ideal for someone with a clinical background and experience in community settings, focused on enhancing patient experiences and outcomes.

Key Responsibilities:
• Coordinate development and documentation of discharge care plans
• Identify goals for continuity of care post-discharge




• Communicate plans effectively to all relevant stakeholders
• Educate patients and families about discharge processes
• Advocate for necessary resources and system improvements

Requirements:
• BScN, BSW, or MSW required
• 2-5 years clinical experience in acute care
• Current registration with respective professional College
• Knowledge of community resources and legislation
• Ability to work independently and with teams
