Unison Health and Community Services seeks a dedicated Community Health Nurse for their Keele-Rogers site. In this full-time contract role, you'll provide primary care assessments, chronic disease monitoring, and health education while working with a multidisciplinary team. Your expertise in clinical primary care and advocacy will be vital to enhancing community well-being.
Key Responsibilities:
• Provide primary wellness assessments and immunization
• Conduct health teaching and advocacy for clients
• Collaborate within a multidisciplinary health team
• Participate in team meetings and center-wide initiatives
• Address community health issues with local service providers
Requirements:
• Licensed as a Registered Nurse in Ontario
• Baccalaureate degree or relevant experience
• Experience with Electronic Health Records
• Current RNAO membership is an asset
• Ability to work evenings and within diverse communities
