Join VHA Home HealthCare as a Community Access Physiotherapist, delivering essential care to patients transitioning from hospital to home in the Toronto Central region. This full time role emphasizes timely, client-focused interventions and interdisciplinary collaboration.
In this key role, you will assess, consult on, and implement physiotherapy interventions for adult patients as part of the UHN@Home Program. Focused on stabilization and functional improvement, the position requires rapid initial assessments within 48 hours post-discharge. You’ll work alongside a Rehabilitation Assistant and connect patients with necessary long-term community support.
Key Responsibilities:
• Assess clients using various evaluation techniques
• Implement interventions based on personalized care plans
• Ensure evidence-based practices and outcomes are documented
• Attend interdisciplinary meetings to discuss patient progress
• Communicate with care team members about intervention plans
Requirements:
• Degree in Physiotherapy from a recognized institution
• Registered with the College of Physiotherapists of Ontario
• 1+ year clinical experience, ideally in community settings
• Clinical expertise in diverse physiotherapy interventions
• Knowledge of community care initiatives and resources
