Become a vital part of IWK Health in Halifax as an Access Navigator in our Mental Health and Addictions program. Support patients and families through effective assessments and resource navigation.
In this temporary full time role, you will report to the Manager and work as part of a comprehensive health care team. Providing empathetic support and accurate clinical assessments, you will guide families to appropriate mental health services and resources within IWK and the community. Knowledge of care pathways and communication skills are essential to facilitate smooth referrals.
Key Responsibilities:
• Collect clinical information from clients and families
• Direct referrals to suitable care pathways
• Conduct telephone facilitated intakes for appointments
• Assess risk and triage client needs
• Collaborate with interprofessional team members
Requirements:
• Bachelor of Social Work and NSCSW registration
• Minimum 2 years of clinical experience
• Knowledge of community resources relevant to care
• NVCI certification and current BLS certification
• Competency in managing patient information systems
