Join Halton Healthcare as a Full-Time Discharge Planner supporting optimal patient transitions across multiple sites. Focus on care coordination and critical discharge planning in a collaborative team environment.
This temporary full-time role spans approximately six months, requiring flexibility with shifts from Monday to Sunday. The Discharge Planner will address discharge barriers, coordinate plans, and communicate effectively with patients and community partners to streamline their transition back to home or institutions.
Key Responsibilities:
• Coordinate care plans for hospital discharge and secure community transition
• Identify goals enhancing continuity of care for patients and families
• Communicate discharge plans to all stakeholders involved
• Ensure understanding of plans by patients and Substitute Decision Makers
• Advocate for resources to fill service gaps
Requirements:
• BScN or BSW or MSW required
• 2-5 years of clinical experience in acute care
• Knowledge of community resources and legislation
• Excellent communication and organizational skills
• Ability to work independently and within a team
