CMHA Peel Dufferin in Brampton is hiring a Transitional Case Manager for the Brampton HART Hub to provide case management within a Housing First framework. You will conduct psychosocial assessments, develop care plans, link clients to community resources, and support transitions from homelessness into housing and recovery.
The role is full-time, based onsite at the Brampton HART Hub, with a multidisciplinary team and opportunities for career development.
