Become a vital part of CommUnityCare as a Community Health Worker, delivering culturally relevant health education to empower local residents. This role involves engaging with patients to improve their health management and ensuring they receive the care they deserve.
As a Community Health Worker, you will collaborate closely with healthcare teams while serving as a cultural mediator. Your focus will be on empowering patients and their families to take active roles in their care, helping them set achievable personal goals and facilitating appointment attendance.
Key Responsibilities:
• Collaborate with care team for patient-centered support
• Provide cultural mediation between patients and healthcare providers
• Empower families to engage in health management
• Follow-up with patients through home visits and calls
• Advocate for community health needs through outreach
Requirements:
• High school diploma or equivalent
• One year of community health experience
• Certified Community Health Worker or related training
• Valid Texas Driver’s License with insurance
• Bilingual Spanish/English preferred
